Registration
Players/Participants
All players/participants are asked to register with USA Hockey prior to submitting your Auburn Youth Hockey Player Registration Form. Once you register with USA Hockey, you will be given the opportunity to print out your receipt. Please bring this receipt with you to the registration. If you can not print the receipt, please write down the registration number that is assigned to your child/children. Players will also need to complete a Code of Conduct form and Consent to Treat/Medical History form available at Registration August 4th and September TBD. These forms may also be downloaded from the USA Hockey site. All documents are required prior to the player stepping onto the ice.

Coaches
Letter to Coaches
Head Coaches
All those applying for head coaching positions are asked to fill out an
Auburn Youth Hockey Coaching Application and
Code of Conduct Form return it to the CEP Director
Dana Berube. Head Coaching positions are then selected by the AYH Board. Once you have been selected as a Head Coach, you will be required to register with USA Hockey (see link above) and submit a copy of your registration to the CEP Director.
Coaches Screening Process
All coaches must go through a screening process every three years, which has been mandated by the state organization (MeAHA). To complete this process go to the
USA Hockey/MeAHA Insurance and Risk Management web site. Scroll down the page and follow the Maine Hockey Online Background Screening Program Instructions.
Once you have completed this process, you must print the confirmation page and turn in a copy to the AYH CEP Director,
Dana Berube.
All documents are required and head coaches must be approved prior to stepping onto the ice.
Assistant Coaches
Once head coaches have been selected, they will be allowed to nominate up to 3 assistant coaches (No more than 4 total coaches can be on the bench during a game). Candidates should have the Auburn Youth Hockey Coaching Application and Code of Conduct Form already filled out and turned in to be considered and approved by the board. Once an assistant coach has been approved, he/she will be asked to register with USA Hockey (see link above) and go through the screening process, which has been mandated by the state organization (MeAHA). The following link will take you to the MeAHA site where you should scroll down until you find the instructions for the screening process. Once you have done this, all paperwork will need to be turned into the CEP Director, Dana Berube. All documents are required and assitant coaches must be approved prior to stepping onto the ice.
