AYH Apparel
Apparel orders will be placed late September, late November (for Christmas delivery), and early January. If you have a large order or any other special needs, arrangements may be made to place a special order.
To place an order, complete the AYH Apparel Order Form, make check payable to Auburn Youth Hockey, return order form and check to Scott Rodrigue, 452 Hotel Road, Auburn, ME 04210. E-mail Scott Rodrigue with any questions.
DO NOT leave order form and check at the concession stand at the rink and DO NOT give to any other AYH members as this could result in lost items or delays in processing.

For each article of clothing add $5.00 for one embroidered/printed name. Add $3.50 for each additional name, division, or jersey #.
Additional embroidery on same article of clothing add $3.50 for each.
Example #1: If you wish to have just the Name, write beside the item your name. Example: Alex. Cost would be $5.00 for each article of clothing.
Example #2: If you wish to have Name and Jersey #, write beside the item. Example: Jeff, #25. Cost would be $8.50 for each article of clothing.
Example #3: If you wish to have Name, Jersey Number, and Division, write beside the item. Example: John, #12, Mites. Cost would be $12.00 for each article of clothing.
If the item is for a coach, indicate "Coach" by the name. Example: Coach Stephen. No extra charge for this.
